Nursing documentation

PICASO nursing documentation is focused on facilitating documentation of those tasks performed at the patient bedside, particularly those that take up nursing time or have traditionally caused the most compliance problems for hospitals due to absent or late charting. The documentation tools are intuitive, simple to use, and seamlessly integrated into nursing workflow, saving nursing staff substantial time.
Because charting is done timely and accurately, it can be accurately used as the basis for future alerts, such as time to round on patient.

Clinical Alerts and Reminders
Utilizing data input from PICASO and other systems, this module identifies and communicates real-time clinical alerts, reminders and other information to help caregivers quickly identify risks to patients and important to do’s.
Caregiver Communication:
Push high priority alerts/reminders to text pagers, smart phones, email, Vocera, Spectralink, etc. Alerts are completely configurable and can be sent directly to the nurse taking care of that patient.
Less urgent alerts, such as reminders to nursing staff to perform compliance-related tasks like restraint monitoring or patient repositioning for decubitus ulcer prevention, are displayed to the caregivers on PICASO’s unique touchscreen view. This is most commonly a touch screen placed in the patient’s room which displays only alerts for that patient; and a nurse unit view placed in one or more locations that is easily visible to caregivers, which displays all of the patients on the unit. New alerts and reminders are prominently displayed where caregivers can easily see them.
Examples of alerts and reminders
Hot button compliance issues:
- Pain alert - Pain reassessment within 30 minutes after narcotic administration.
- Restraint alert - q 2 hour restraint monitoring due
- Skin assessment alert – patient at risk for skin breakdown, and q 12 hour re-assessment is past due
Never event prevention:
- Patient has been identified at risk for skin breakdown, and it’s time to turn the patient and perform skin care.
- Fall alert - patient is at risk for fall, AND bed exit alarm is not set on the bed
- VAP alert – patient is on a ventilator AND the head of the bed is <30 degrees
Other:
- Critical lab result alert
- Core measures alert - Beta blocker not administered within 1 hour of admission for potential core measures MI case
- Rapid response team alert - BP >x and HR>x, or delta change in respiratory rate > x
- Observation alert – 23 hour observation patient is approaching/exceeding 23 hours stay
Data is integrated from multiple sources, as many or as few as hospital prefers: nurse call data, smart bed data, ADT, results, orders, meds admin, nursing documentation, etc.
